2) Consent to Treat

Go to USAHockey.org and choose Membership on the navigation bar across
the top. I think the fee is $35. Please forward your email

5) Parent Code of Conduct

1) Background Screening (copy of email confirmation)

Go to MAHA.org and choose Forms & Info across the top, then choose
Background Check on the left-hand side. According to the MAHA
website, they have included the Coaches Ethics Agreement within the
Background check so that is no longer necessary to complete
separately. Please forward your email confirmation to your manager for their rostering package

Behind The Bench

Please make sure to:

  • Verify you are at the correct CEP level and if not, get there by 12/31/2008.   No waivers this year.   Here is a link to the USA Hockey website.
  • See the Parents tab for instructions to register yourself with USA hockey for insurance purposes.
  • Go to https://www.maha.org/CoachingBackgroundCheck.aspx to fill out the MAHA background check.

MAHA notes and rules:

  • Coaches must bring their cards to every game.
  • Coaches must sign every score sheet.
  • There should be no parent/player games or non-rostered players on the ice or behind the bench during practices or games.  These situations invalidate  insurance.
  • USA Hockey & MAHA have revised the Helmet Rule
    to include independent contractors/instructors:

RISK MANAGEMENT

All ice hockey coaches and instructors of registered USA Hockey youth, high school, disabled, girls/women’s 19&under and below programs must wear an ice hockey helmet during all on ice sessions, including practices, controlled scrimmages and coach and referee clinics (seminars), with the exception of when a team official is tending to an injured player. Any team personnel found in violation of this rule by their District Council shall be subject to a recommended suspension of not less than 30 days, compromise their responsibilities or reduce the public's trust in coaching and/or coaches. Coaches are concerned about the ethical compliance of their colleagues' conduct. When appropriate, they consult with their colleagues in order to prevent or avoid unethical conduct.
